'24 Mailbox Project • Key Release Authorization

By submitting this form, the homeowner authorizes the Chattahoochee Terrace Homeowners Association (CTHA), Access Management Group (AMG), and Mailbox Project LLC to release the new mailbox key to the individual listed below. Upon submission, the information is sent to AMG for key distribution.

Note: The vendor requires ID for key pickup; the name must match the owner in AMG records or the person listed on this form​.    

KEY RELEASE AUTHORIZATION

I, the CTHA homeowner submitting this form, authorize CTHA and its agents to release the new mailbox key(s) for my property to the tenant(s) named on this form.   I understand it is my responsibility to inform my tenant(s) of the key distribution schedule as announced via email by AMG and the requirement to provide a government ID for name verification to receive the keys.

By completing and submitting this form, I confirm that I am the legal owner of the property, and this authorization is for the sole purpose of releasing the mailbox key(s) to my tenant(s). I acknowledge that my tenant must comply with this authorization and any CTHA rules and regulations.
